To our valued patients,

Under the guidance of the CDC and the Colorado Dental Association, we assure you that the safety of our dental families and team is our top priority.

♥ In order to check-in, please call our office (720-886-0808) and the front desk will check you in.

♥ Please remain in your car and we will call you back when we are ready to have you seated.

♥ A new COVID-19 Waiver form and questionnaire will be given to you to read and sign.

♥ We will update your insurance and personal information during this call, if needed.

♥ We ask that only one parent/guardian accompany the patient to the appointment. Other siblings are respectfully asked to stay home.

♥ All parents must wear a mask to appointments.

♥ Once you are called to enter, you will be greeted in the waiting area and your temperatures will be taken. Anyone with a temperature over 100.4 degrees will not be seen.
